Our Social Services Department is a primary link between the nursing center and the family. The staff offers support, answers questions and assists with all medical issues while the resident is in the facility. Our Social Services staff conducts regular Orientation Sessions for new residents and family members. They also lead support groups and education sessions for family members and are available for private counseling. The Social Services Director also works with the patient, family, staff, and physician to determine a discharge date. In order to plan a safe and secure transition to home, we will go to the patient's residence to assess the environment. After the move home, we keep in touch with the patient and the family to help if follow up assistance is needed. |









The transition from home to a skilled nursing facility requires personalized attention. Our Social Services Department is trained in counseling and assistance. They are the first to greet the resident upon arrival. They are committed to getting to know all residents and to helping them adjust to residing in a new place.
